The partnership integrates EHR systems with real-time clinical data from thousands of providers with Altruista’s GuidingCare® medical management and population health platform. The integration will enable payers and providers to share real-time data, enhancing care coordination while also reducing the administrative burden on providers.

Altruista’s GuidingCare platform will integrate with clinical data from 3,000-plus providers in 48 states. Payers will know immediately when a member is being discharged from the hospital, for example, so the payer can help coordinate post-discharge care. This will reduce readmissions and gaps in care, effectively improving quality and reducing medical spending. Until now, payers had to rely on providers to notify them of patient events, often creating a lag time of days or weeks.

Halfpenny Technologies offers cloud-based, fully managed solutions that support care coordination, care management, regulatory requirements and data analysis within and between healthcare organizations. They link hospitals, labs, health plans, pharmacies and HIEs. With this partnership, Halfpenny Technologies now populates GuidingCare with this data.
